• No Objection Certificate (in original) of the institution passed by the candidate for MBBS/BDS examination.
• No Objection Certificate (in original) of the college/institution from which the candidate has to do the internship.
• Photocopy of Provisnal Registration Certificate.
• Photocopy of Aadhar Card.
• Marksheet of Final Professional of MBBS Course.
• Affidavit is not Requried For Government Student.
• Affidavit on stamp paper of ₹ 10/- for undertaking Unpaid Internship.
